Before Damyan can create your special unique portrait we need you to submit this completed form to have an idea of your needs and preferences. If you want a portrait of a CELEBRITY other than the examples in our GALLERY please let us know their name and complete also the relevant fields below. If you have a specific photo you want to be used please send it.
If you want a portrait of YOURSELF or a friend please send at least one photo by email or normal post with a completed copy of this form (our address is in the contact link on the main site).

If you do not want the artist to create a COPY of the photo please send several and as much info about the person as possible so Damyan can create a portrait in his own style and use his imagination as he has with the many CELEBRITY PORTRAITS! We can also do portraits with 2 – 3 persons!
